United States law requires a Medical Examination for Immigration, performed by doctors who are approved for such an examination (also known as “civil surgeon”).
The authorized physician must complete Form I-693 with the test results along with immunization verification. We understand that the immigration process and medical examination is an ordeal. To help simplify this, we are pleased to offer our complete immigration examination service as an authorized Civil Surgeon at My Family Doc, this includes:
A medical exam by the Civil Surgeon
Vaccination/Immunization review
Required Lab tests for:
Tuberculosis
Syphilis and Gonorrhea – ages 18 and older

Vaccination/immunization services are an additional cost. These are determined after the Civil Surgeon has reviewed your medical history. For more information, you may refer to Vaccination Requirements
Patients are advised to present the following during their appointment:
A current, unexpired and valid photo ID – acceptable forms are government-issued Driver’s License, State Identification or Passport
A pre-completed USCIS Form I-693. Along with filling instructions, this may be downloaded directly from the USCIS website: https://www.uscis.gov/i-693
Shot records (vaccination/immunization history). These must be officially translated in English and accomplished by an appropriate medical personal. Self-reported vaccine doses without written documentation are not acceptable
Any significant medical records pertaining to mental health, substance abuse or previous infection for Tuberculosis, Syphilis or Gonorrhea
*Immigration medical exam service is not typically covered by insurance. Patients may opt to claim payment from their insurance by submitting a receipt for our services. However, we cannot guarantee reimbursement. We accept payment in cash, check or credit card. Payment is due at the time of service.
Effective October 1, 2021, COVID-19 Vaccination is required for all applicants, 12 years old and over.
